AB OBJECTIVE: In order to assess the diagnostic value of urine testing in
HIV seropositive (SP) individuals, we examined urine and serum specimens
in parallel to peripheral blood lymphocyte (PBL) populations from 74 HIV
SP subjects. METHODS: Urine specimens were tested by EIA Abbott 3rd
generation (3-G-EIA) and modified EIA (Mod-EIA). 156 seronegative
subjects served as controls. Positive results were confirmed by Western
blot (WB). For Mod-EIA, mouse antihuman IgG (Sigma) was substituted as
the conjugate in the Abbott kit. PBL were monitored in a FACScan (BD)
using monoclonal antibodies (Coulter) for T-cells CD3, CD4 and CD8.
RESULTS: Sixty-nine out of 74 urine samples of SP individuals reacted in
3-G-EIA, whereas all 74 reacted in Mod-EIA. All five cases reacting as
false negative urine samples in 3-G-EIA had CD4 counts higher than
900/mm2), CD4/CD8 ratio > 1 and indeterminate WB patterns with bands to
gp160 or p24. The urine OD cut-off ratio in HIV SP subjects with CD4
counts above 700 was significantly lower than that of all other groups
(p < 0.005). Sensitivity and specificity of Mod-EIA were 100% and 93.6%
respectively. DISCUSSION AND CONCLUSION: Urinary antibody using Mod-EIA
with 3-G-EIA is a simple highly sensitive and specific test for
screening HIV infection and assessing early stages of HIV
seroconversion.
